Did you see the move Cast Away starring Tom Hanks? If so, you probably remember the scene where Hanks, stranded on a remote island, knocks out his own abscessed tooth — with an ice skate, no less — to stop the pain. Recently, Dear Doctor TV interviewed Gary Archer, the dental technician who created that special effect and many others.
“They wanted to have an abscess above the tooth with all sorts of gunk and pus and stuff coming out of it,” Archer explained. “I met with Tom and I took impressions [of his mouth] and we came up with this wonderful little piece. It just slipped over his own natural teeth.” The actor could flick it out with his lower tooth when the time was right during the scene. It ended up looking so real that, as Archer said, “it was not for the easily squeamish!”
That’s for sure. But neither is a real abscess, which is an infection that becomes sealed off beneath the gum line. An abscess may result from a trapped piece of food, uncontrolled periodontal (gum) disease, or even an infection deep inside a tooth that has spread to adjacent periodontal tissues. In any case, the condition can cause intense pain due to the pressure that builds up in the pus-filled sac. Prompt treatment is required to relieve the pain, keep the infection from spreading to other areas of the face (or even elsewhere in the body), and prevent tooth loss.
Treatment involves draining the abscess, which usually stops the pain immediately, and then controlling the infection and removing its cause. This may require antibiotics and any of several in-office dental procedures, including gum surgery, a root canal, or a tooth extraction. But if you do have a tooth that can’t be saved, we promise we won’t remove it with an ice skate!
The best way to prevent an abscess from forming in the first place is to practice conscientious oral hygiene. By brushing your teeth twice each day for two minutes, and flossing at least once a day, you will go a long way towards keeping harmful oral bacteria from thriving in your mouth.

However, during an exclusive interview with Dear Doctor magazine, she shared her experiences with cosmetic dentistry. “I had a bridge put in probably 30 years ago where I had a tooth pulled and there was a space,” Vanna said.
Prior to having a permanent tooth pulled, most people are concerned with what can be done to replace it. It's important to follow through and do exactly that. This is especially true with back teeth. Just because you can't see them, it doesn't mean you won't face problems if they are not replaced.
For example, did you know that missing posterior (back) teeth can lead to a wide array or problems with the remaining teeth, muscles, ligaments, joints and jaw bones? This includes:
- A decrease in chewing efficiency that in turn can impact your diet, nutrition and overall health
- Excessive erosion or wear of remaining teeth
- Tipping, migration, rotation and even loss of adjacent remaining teeth
- Painful jaw problems such as Temporomandibular Dysfunction (TMD)

Fans of the primetime TV show The Middle were delighted to see that high school senior Sue, played by Eden Sher, finally got her braces off at the start of Season 6. But since this popular sitcom wouldn’t be complete without some slapstick comedy, this happy event is not without its trials and tribulations: The episode ends with Sue’s whole family diving into a dumpster in search of the teen’s lost retainer. Sue finds it in the garbage and immediately pops it in her mouth. But wait — it doesn’t fit, it’s not even hers!
If you think this scenario is far-fetched, guess again. OK, maybe the part about Sue not washing the retainer upon reclaiming it was just a gag (literally and figuratively), but lost retainers are all too common. Unfortunately, they’re also expensive to replace — so they need to be handled with care. What’s the best way to do that? Retainers should be brushed daily with a soft toothbrush and liquid soap (dish soap works well), and then placed immediately back in your mouth or into the case that came with the retainer. When you are eating a meal at a restaurant, do not wrap your retainer in a napkin and leave it on the table — this is a great way to lose it! Instead, take the case with you, and keep the retainer in it while you’re eating. When you get home, brush your teeth and then put the retainer back in your mouth.
If you do lose your retainer though, let us know right away. Retention is the last step of your orthodontic treatment, and it’s extremely important. You’ve worked hard to get a beautiful smile, and no one wants to see that effort wasted. Yet if you neglect to wear your retainer as instructed, your teeth are likely to shift out of position. Why does this happen?
As you’ve seen firsthand, teeth aren’t rigidly fixed in the jaw — they can be moved in response to light and continuous force. That’s what orthodontic appliances do: apply the right amount of force in a carefully controlled manner. But there are other forces at work on your teeth that can move them in less predictable ways. For example, normal biting and chewing can, over time, cause your teeth to shift position. To get teeth to stay where they’ve been moved orthodontically, new bone needs to form around them and anchor them where they are. That will happen over time, but only if they are held in place with a retainer. That’s why it is so important to wear yours as directed — and notify us immediately if it gets lost.
And if ever you do have to dig your retainer out of a dumpster… be sure to wash it before putting in in your mouth!

She's an international star who's recognized everywhere she goes. As Carol Brady, she was an ambassador for the “blended family” before most of us even knew what to call her bunch. And her TV Land Pop Culture Icon Award is on permanent display in the National Museum of American History. So what item that fits inside a purse can't Florence Henderson do without?
“I will never leave home without dental floss!” she recently told an interviewer with Dear Doctor magazine. “Because I have such a wide smile, I have found spinach or black pepper between my teeth after smiling very broadly and confidently.”
Henderson clearly understands the importance of good oral hygiene — and she's still got her own teeth to back it up! In fact, flossing is the best method for removing plaque from between the teeth, especially in the areas where a brush won't reach. Yet, while most people brush their teeth regularly, far fewer take the time to floss. Is there any way to make flossing easier? Here are a couple of tips:
Many people have a tendency to tighten their cheeks when they're holding the floss, which makes it more difficult to get their fingers into their mouths and working effectively. If you can relax your facial muscles while you're flossing, you'll have an easier time.
To help manipulate the floss more comfortably, try the “ring of floss” method: Securely tie the floss in a circle big enough to easily accommodate the fingers of one hand. To clean the upper teeth, place fingers inside the loop, and let the thumb and index finger guide the floss around each tooth. For the lower teeth, use two index fingers. Keep moving the floss in your hand so you always have a clean edge... and remember, the goal is to get the tooth clean, but it shouldn't hurt — so don't use too much pressure or go too fast.

Here's an interesting tidbit of information on Wheel of Fortune host Vanna White: like many people, she grinds her teeth at night. In a detailed interview with Dear Doctor magazine, Vanna explained how she had to replace a filling in a back tooth several times because of her grinding habit. Eventually, she had her dentist make her a nightguard to protect her famous smile.
“I really try to sleep with it every night,” Vanna told the magazine. “I try to keep it on my nightstand so when I go to bed, I remember to put it in. Or I will put it by my toothbrush so I can put it in after brushing my teeth at night.”
The habit of teeth grinding or clenching is often associated with stress and/or sleep deprivation. It is referred to as “parafunctional” (“para” – outside, “function” – normal), meaning it can generate biting forces well outside the normal range — perhaps 10 times normal. This excessive force can affect many areas of the oral system. Teeth may become worn, chipped or loose; jaw joints or muscles can go into spasm; and some grinders (or “bruxers” as they are also called) may even experience discomfort of the head, ears, neck or back. Many times, a person with a grinding habit does not become aware of it until it is pointed out by a sleep partner or dental professional.
Like Vanna White's dentist, we often recommend a nightguard to those with nocturnal bruxing habits. It is made of a very thin, wear-resistant plastic that fits over the biting surfaces of the upper teeth only. The lower teeth are then free to glide or skate over the guard, which prevents them from biting into the upper teeth. Some people wear their guards during the day if they tend to clench their teeth when under stress.
